Well this is your first visit. Congratulations!!

You have taken the first step toward recovery  and a healthier way of living.
Great, lets get you started.

There are a few steps that we will go through first.

Step 1 :   We will need a referral from your physician and this can be faxed to us at 619-425-1858 in the                         Bonita Office or 619-223-7030 in Point Loma to get things started, but  we will need the                         original when you come in.

Step 2:    You may speak to Synthia, "our insurance verifier", in the Bonita Office, or Caroline in the Point                         Loma Office, to make sure you're completely covered for your  visits.  If there                         are any problems we should know how to contact you.

Step 3:    After your insurance is verified, then we can schedule you for your Initial Eval.  For this you will
                        need to bring your insurance card, your original prescription, and your ID.   

Step 4:    Now we will need  you to fill out some paper work including personal information and  patient                         history. Now you are ready for you Eval.
